{"id":11480,"date":"2011-07-13T09:43:52","date_gmt":"2011-07-13T13:43:52","guid":{"rendered":"http:\/\/1boringoldman.com\/?p=11480"},"modified":"2011-07-13T11:44:21","modified_gmt":"2011-07-13T15:44:21","slug":"go-pogo","status":"publish","type":"post","link":"https:\/\/1boringoldman.com\/index.php\/2011\/07\/13\/go-pogo\/","title":{"rendered":"go pogo&#8230;"},"content":{"rendered":"\n<div align=\"justify\">It&#8217;s an absurd paper, long forgotten &#8211; just another one of those CV fillers that were so common ten years ago in the psychiatric literature, and it&#8217;s authored by the now infamous Dr. Nemeroff when he was the &quot;boss of bosses.&quot; In the scope of things &#8211; meaningless drivel&#8230;    <\/div>\n<blockquote>\n<div align=\"center\"><a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed?term=11384898[uid]\" target=\"_blank\"><u><strong><font color=\"#200020\">Double-blind, placebo-controlled comparison of imipramine and paroxetine in the treatment of bipolar depression<\/font><\/strong><\/u><\/a><br \/>     <sup>by Nemeroff CB, Evans DL, Gyulai L, Sachs GS, Bowden CL, Gergel IP, Oakes R, and Pitts CD.<\/sup><br \/>     <strong><font color=\"#200020\">American Journal of Psychiatry<\/font><\/strong>. 2001 158(6):906-12.<\/div>\n<p>     <\/p>\n<div align=\"justify\"><strong>Abstract<br \/>    <sup><u>OBJECTIVE<\/u>: This  study compared the efficacy and safety of paroxetine and imipramine  with that of placebo in the treatment of bipolar depression in adult  outpatients stabilized on a regimen of lithium.<br \/>      <u>METHOD<\/u>: In  a double-blind, placebo-controlled study, 117 outpatients with  DSM-III-R bipolar disorder, depressive phase, were randomly assigned to  treatment with paroxetine (N=35), imipramine (N=39), or placebo (N=43)  for 10 weeks. In addition to lithium monotherapy, patients may have  received either carbamazepine or valproate in combination with lithium  for control of manic symptoms. Patients were stratified on the basis of  trough serum lithium levels determined at the screening visit (high:  &gt;0.8 meq\/liter; low: &lt;=0.8 meq\/liter). Primary efficacy was  assessed by change from baseline in scores on the Hamilton Rating Scale  for Depression and the Clinical Global Impression illness severity  scale.<br \/>      <u>RESULTS<\/u>: Differences in overall efficacy among the  three groups were not statistically significant. For patients with high  serum lithium levels, antidepressant response at endpoint also did not  significantly differ from placebo. However, both paroxetine and  imipramine were superior to placebo for patients with low serum lithium  levels. Compared to imipramine, paroxetine resulted in a lower incidence  of adverse events, most notably emergence of manic symptoms.<br \/>      <u>CONCLUSIONS<\/u>:  Antidepressants  may not be useful adjunctive therapy for bipolar depressed patients  with high serum lithium levels. However, antidepressant therapy may be  beneficial for patients who cannot tolerate high serum lithium levels or  who have symptoms that are refractory to the antidepressant effects of  lithium.<\/sup><\/strong><\/div>\n<\/blockquote>\n<div align=\"justify\">Last night, when I read <a href=\"http:\/\/pogoblog.typepad.com\/pogo\/2011\/07\/amy-gutmann-do-the-right-thing-by-president-obama-be-a-leaderand-resign.html\" target=\"_blank\"><u><strong><font color=\"#400040\">Paul Thacker&#8217;s article in POGO<\/font><\/strong><\/u><\/a> that began&#8230;<\/div>\n<ul>\n<div align=\"justify\"><sup>&quot;Yesterday, POGO sent a letter to President Obama asking that <strong><a href=\"http:\/\/www.upenn.edu\/pennnews\/amy-gutmann\" target=\"_blank\">Dr. Amy Gutmann<\/a><\/strong> be removed as chair of the commission that advises the President on bioethics. We did so because Dr. Gutmann has ignored <strong><a href=\"http:\/\/www.dailypennsylvanian.com\/article\/psychiatry-chairman-faces-ghostwriting-accusations\" target=\"_blank\">serious allegations of research misconduct<\/a><\/strong> regarding a senior professor at the University of Pennsylvania, where she is President&#8230;&quot;<\/sup><\/div>\n<\/ul>\n<div align=\"justify\">&#8230; and looked over the documents, I didn&#8217;t feel the immediate rush of righteous indignation that I usually feel when such things surface. Maybe I was sleepy, or ready for a vacation, I thought. It is a dumb article by a known offender [Charlie Nemeroff], ghost-written by an exposed ghost [Sally Laden], at an exposed ghost-writing firm [STI], hired by a sleazy pharmaceutical company [GSK AKA SKB], in a time of great sickness [2001]. The current issue is a complaint filed by Dr. Jay Amsterdam that he was excluded from authorship on this paper, and later learned that it was ghost-written and created by STI. I think my non-reaction had to do with the adage that if two dogs get into a fight, don&#8217;t try to separate them if you value your life. This issue arose as a squabble between professors over who was in charge of this study at Penn. This morning, I reread the documents, including this old response to Dr. Amsterdam&#8217;s complaint at the time all of this was happening:<\/div>\n<div align=\"center\"><img loading=\"lazy\" decoding=\"async\" width=\"450\" vspace=\"5\" height=\"626\" border=\"0\" src=\"http:\/\/1boringoldman.com\/images\/penn.gif\" \/><\/div>\n<div align=\"justify\">That&#8217;s horrid! I think my non-reaction last night was scandal fatigue. I must need a vacation! And I&#8217;m glad Paul Thacker at POGO doesn&#8217;t have scandal fatigue. Last November, he <a href=\"http:\/\/www.pogo.org\/pogo-files\/letters\/public-health\/ph-iis-20101129.html\" target=\"_blank\"><u><strong><font color=\"#200020\">exposed<\/font><\/strong><\/u><\/a> an egregious example of ghost-writing by Dwight Evans, Chairman at Penn, and nothing happened. So here&#8217;s another example, and the Penn President has done nothing, yet is on Obama&#8217;s Bioethics Commission!<\/div>\n<p align=\"justify\">So take a look at the <a href=\"http:\/\/pogoblog.typepad.com\/pogo\/2011\/07\/amy-gutmann-do-the-right-thing-by-president-obama-be-a-leaderand-resign.html\" target=\"_blank\"><u><strong><font color=\"#660033\">POGO article and the attached documents<\/font><\/strong><\/u><\/a>, feel the outrage, and fire off a letter to anyplace you think might listen [mine went to the <strong><font color=\"#006600\">American Journal of Psychiatry<\/font><\/strong>]. Just because it&#8217;s a dumb article, by a known offender [Charlie Nemeroff], ghost-written by  an exposed ghost [Sally Laden], at an exposed ghost-writing firm [STI],  hired by a sleazy pharmaceutical company [GSK AKA SKB], in a time of  great sickness [2001], doesn&#8217;t mean it should be ignored. <\/p>\n<div align=\"justify\">Paul Thacker&#8217;s got the right idea. The people in high places that don&#8217;t act to protect the integrity of academic institutions are as guilty as the offenders. What else are they for, if it&#8217;s not that?
