{"id":5111,"date":"2011-03-01T19:31:57","date_gmt":"2011-03-02T00:31:57","guid":{"rendered":"http:\/\/1boringoldman.com\/?p=5111"},"modified":"2011-03-01T19:31:57","modified_gmt":"2011-03-02T00:31:57","slug":"selling-seroquel-vii-indication-sprawl","status":"publish","type":"post","link":"https:\/\/1boringoldman.com\/index.php\/2011\/03\/01\/selling-seroquel-vii-indication-sprawl\/","title":{"rendered":"selling seroquel VII: indication sprawl&#8230;"},"content":{"rendered":"\n<p align=\"justify\">Most of the <a target=\"_blank\" href=\"http:\/\/psychrights.org\/research\/Digest\/NLPs\/Seroquel\/090520UnsealedSeroquelExhibits\/DocumentDescriptionTables.pdf\"><strong><font color=\"#200020\">internal documents<\/font><\/strong><\/a> from <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> relate to the first half of the decade when the focus was on the competition among the Atypicals based on efficacy and adverse effects treating Schizophrenia &#8211; EPS, TD, weight gain, Diabetes, etc. By mid-decade [CATIE], clinical experience and the longer-term studies had pretty well clarified things for anyone who paid attention. Zyprexa was the most efficacious but caused the greatest weight gain and risk for Diabetes. Risperdal usually came in second in efficacy, but caused the most EPS and TD. Seroquel was the Atypical most likely to be discontinued, though in the middle in terms of side effects. None was the side effect free, potent antipsychotic of the ads [or our dreams]. And by 2004, the F.D.A. was requiring labeling warnings for the Metabolic Syndrome [Diabetes] and Tardive Dyskinesia, and was beginning to issue <a target=\"_blank\" href=\"http:\/\/www.pharmcast.com\/WarningLetters\/Yr2005\/Mar2005\/AstraZeneca0305.htm\"><u><strong><font color=\"#200020\">warning letters<\/font><\/strong><\/u><\/a> for off-label promotion.<\/p>\n<p align=\"justify\"><strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> had planned to expand its indications into the Bipolar Disorders in the  <a href=\"http:\/\/psychrights.org\/research\/Digest\/NLPs\/Seroquel\/090520UnsealedSeroquelExhibits\/Off-label\/Schwartz%207%20%5BRedacted%5D%20-%20Seroquel%20Strategy%20Summary%20%2812-18-00%29.pdf\" target=\"_blank\"><u><strong><font color=\"#6600ff\">Seroquel<\/font><\/strong> <strong><font color=\"#400040\">Strategic Plan<\/font><\/strong><\/u><\/a> of 2000, and had been doing that in &quot;off-label&quot; promotions. But when the warning letter came and <strong><font color=\"#000099\">Zypreza<\/font><\/strong> was approved by the F.D.A. for Mania and Bipolar treatment, they initiated a number of clinical trials of <strong><font color=\"#6600ff\">Seroquel<\/font><\/strong> in Manic and Bipolar Disorder patients [referred to by some as the &quot;<em><strong><font color=\"#200020\">me too<\/font><\/strong><\/em>&quot; phenomenon]. Their approval soon followed. Approvals varies from drug to drug, this chart captures the gist of the time-line of the approval for their various indications.<\/sup><\/td>\n<\/tr>\n<\/table>\n<p align=\"justify\">People acquainted with how all this works immediately recognize the Clinical Trial formula used in getting these F.D.A. Approvals [described earlier &#8211; <strong><u><a target=\"_blank\" href=\"http:\/\/1boringoldman.com\/wp-admin\/..\/index.php\/2011\/02\/19\/selling-seroquel-iii-the-data-factories\/\"><font color=\"#200020\">selling seroquel III: the data factories&hellip;<\/font><\/a><\/u><\/strong>].They all look the same: some rating scale plotted against time comparing <strong><font color=\"#6600ff\">Seroquel<\/font><\/strong> to placebo. The placebo group always gets better, and the drug adds a little something, sometimes more than others, but always significant. There are usually 50% drop-out rates, sometimes more. And the ITT\/LOCF methodology makes the primary data opaque.    <\/p>\n<p align=\"center\"><img decoding=\"async\" border=\"0\" src=\"http:\/\/1boringoldman.com\/images\/sell-seroquel-beach-8.gif\" \/><\/p>\n<p align=\"justify\">These clinical trials are funded by <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong>, have a number of <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> employees as authors, are conducted by CRO&#8217;s selected and paid for by <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong>, and are written by a medical writers hired by <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong>. But the Psychiatrist authors were in the mix, recruited by <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> as well. Here&#8217;s an example where one of the &quot;invited&quot; authors balked:<\/p>\n<p align=\"justify\">Dr. Roger McIntyre suggested Dr. <a href=\"http:\/\/nassirghaemi.com\/\" target=\"_blank\"><u><strong><font color=\"#200020\">Nassir Ghaemi<\/font><\/strong><\/u><\/a> as a second author of a Clinical Trial paper comparing Haldol and Seroquel in treating Manic episodes. Obviously, the trial was over and the paper almost finished by the time he was asked. [To his credit] Dr. Ghaemi responded with:   <\/p>\n<p align=\"center\"><a href=\"http:\/\/psychrights.org\/research\/Digest\/NLPs\/Seroquel\/090520UnsealedSeroquelExhibits\/Transparency%20-%20Concealing\/Piano17.pdf\" target=\"_blank\"><img loading=\"lazy\" decoding=\"async\" width=\"520\" height=\"355\" border=\"0\" src=\"http:\/\/1boringoldman.com\/images\/sell-seroquel-beach-9.gif\" \/><\/a><\/p>\n<p>This is an internal <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> email:  <\/p>\n<p align=\"center\">&nbsp;<a href=\"http:\/\/psychrights.org\/research\/Digest\/NLPs\/Seroquel\/090520UnsealedSeroquelExhibits\/Transparency%20-%20Concealing\/Piano17.pdf\" target=\"_blank\"><img decoding=\"async\" border=\"0\" src=\"http:\/\/1boringoldman.com\/images\/sell-seroquel-beach-11.gif\" \/><\/a><\/p>\n<p align=\"justify\">And one of the responses:<\/p>\n<p align=\"center\"><a href=\"http:\/\/psychrights.org\/research\/Digest\/NLPs\/Seroquel\/090520UnsealedSeroquelExhibits\/Transparency%20-%20Concealing\/Piano17.pdf\" target=\"_blank\"><img decoding=\"async\" border=\"0\" src=\"http:\/\/1boringoldman.com\/images\/sell-seroquel-beach-13.gif\" \/><\/a><\/p>\n<p align=\"justify\">The paper was published in 2005 with Dr. McIntyre and four <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> employees as authors [no Dr. Ghaemi] [<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16139175\" target=\"_blank\"><u><strong><font color=\"#200020\">Quetiapine or haloperidol as monotherapy for bipolar mania &#8211; 12-week, double-blind, randomised, parallel-group, placebo-controlled trial<\/font><\/strong><\/u><\/a>]:<\/p>\n<p align=\"center\"><img decoding=\"async\" border=\"0\" src=\"http:\/\/1boringoldman.com\/images\/sell-seroquel-beach-10.gif\" \/><\/p>\n<p align=\"justify\">[Standard Clinical Trial fare<em> ITT<\/em>, <em>LOCF<\/em>, <em>50%+ drop-out<\/em>] In case you&#8217;re wondering why the two people receiving the internal <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> email are in <strong><font color=\"#990000\">red<\/font><\/strong>, it&#8217;s because they&#8217;re the contract writers from <a href=\"http:\/\/www.parexel.com\/\" target=\"_blank\"><u><strong><font color=\"#200020\">Parexel<\/font><\/strong><\/u><\/a> who actually wrote the paper:<\/p>\n<p align=\"center\"><img loading=\"lazy\" decoding=\"async\" width=\"520\" height=\"41\" border=\"0\" src=\"http:\/\/1boringoldman.com\/images\/sell-seroquel-beach-12.gif\" \/><\/p>\n<div align=\"justify\">I&#8217;ve almost worn out my keyboard writing my little series of series [<strong><font color=\"#200020\">Mifepristone<\/font><\/strong>,<strong><font color=\"#200020\"> Evidence Based Medicine<\/font><\/strong>,<strong><font color=\"#200020\"> Seroquel<\/font><\/strong>,<strong><font color=\"#200020\"> Selling Seroquel<\/font><\/strong>] trying to get something said and find a way to say it. More than any of my examples, this one says it best. This is something of a trivial Clinical Study. Had any Psychiatrist reading this been asked in advance, &quot;How will Seroquel do against Haldol in treating Acute Mania?&quot;, they would&#8217;ve likely answered, &quot;It&#8217;ll work, but not as well as Haldol.&quot; But that&#8217;s not the point. The point is that a KOL [Dr. Ghaemi] was asked to be the second author of a paper, and he said he&#8217;d do it if he could actually see the raw data. Essentially, they bounced that request off of each other around <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong> and said &quot;No.&quot; That is almost too outrageous to believe, that an author actually wanted to see the data, analyze it himself, and they saw that as a threat. Not only that, the internal bouncing around included the two contract writers at <a href=\"http:\/\/www.parexel.com\/\" target=\"_blank\"><u><strong><font color=\"#200020\">Parexel<\/font><\/strong><\/u><\/a> who conducted the Clinical Trial in the first place and likely wrote the whole paper &#8211; yet another formulaic clinical trial graph that looks like all the others. As I perused the Clinical Trials that got <strong><font color=\"#660099\">Serpquel <\/font><\/strong>approved initially, then approved for the various indications, I had the fantasy that I could cut and paste the sections from the various papers and interchange them, and no one would notice &#8211; or that I could write a computer program that would generate the studies themselves. We owe Dr. Ghaemi a debt of gratitude for responding [like all those other add-on authors should have], and in doing so letting us see <strong><font color=\"#660033\">AstraZeneca<\/font><\/strong>&#8216;s internal response &#8211; &quot;If we have to let you authors see the data, we&#8217;ll have our people go over it with you.&quot; Absolutely outrageous! Evidence based medicine?
